I understand my responsibility as occupant and accept financial burden for any damages to Mammoth Snowcreek Rental caused by me or any member of my party. By signing below, I agree to pay the owners for repairs to Mammoth Snowcreek Rental in the event damages are caused by us during our stay; and I agree to pay replacement value for any items which are broken or lost to theft during my stay. Such damages will be reported in writing to me by the owners.

Please observe the following rules in order to make your stay more pleasant: This property is privately owned, and the management reserves the right to refuse service to anyone. The management will not be responsible for accidents or injuries to guests or for loss of money, jewelry, or other valuables during their stay. Should litigation arise concerning provisions of the agreement, reasonable legal fees will be awarded to prevailing party.

Note: Please let us know if anything in the home does NOT operate properly during your stay.

If guests arrive with more people in their party than the house allows, NO ONE will be permitted to enter the house, and NO REFUND will be issued. California Penal Code Section 537 makes it a crime to defraud the innkeeper, and having more people in the house than have registered is included in the provisions of this code. Maximum of eight (8) overnight occupants and parking for a maximum of two (2) cars at any one time.

Sorry, but NO SMOKING is permitted in the house We request that shoes & ski boots be removed outside and not be worn inside of home. To ensure the overall quality and appearance of MAMMOTH SNOWCREEK RENTAL, these policies are strictly enforced.

Guests staying past 10 A.M. check-out time will be charged a minimum of one-half (1/2) the daily rental rate, and only if there is not another group arriving after you.

Map, Info and Directions: Prior to your arrival at our home in Mammoth, you will receive an information sheet that should answer all your questions and include a map with driving directions.

Transient Occupancy Use Notice of the

I ____________________, having registered as occupant of MAMMOTH SNOWCREEK RENTAL at 4711 Minaret Road #475 for the nights of _________________ declare under penalty of perjury that not more than eight (8) persons including myself will be using MAMMOTH SNOWCREEK RENTAL for overnight sleeping purposed on those nights.

I understand that the Municipal Code of the Town of Mammoth Lakes prohibits transient occupants of lodging facilities from housing additional guests in a unit and that a violation of the provisions of this chapter is a violation of the Municipal Code subject to a civil penalty of not less than one hundred dollars ($100) for each day on which such violation occurs. I further understand that California Penal Code Section 537 prohibits the defrauding of innkeepers and that such fraud is punishable by a fine of up to one thousand dollars ($1,000) and/or by imprisonment in the State prison for up to three (3) years. I further understand that I am signing this declaration under penalty of perjury and that perjury is punishable under California Penal Code Section 126 by imprisonment for a period of from two (2) to four (4) years.

I declare that I have read the above provisions and that I sign this declaration with the full knowledge and understanding of the consequences of any violation of the laws noted above.
